About the Role

We’re looking for an experienced and compassionate Community Registered Nurse – Care Manager to join our dedicated team at Leora, working across the community. This is a dynamic, fields-based role that involves travelling to client’s homes and community settings to deliver high-quality clinical care and case management. You’ll support individuals living with disability, recovering from injury, or requiring age-related care, ensuring their needs are met through personalised, person-centred planning in line with NDIS and Home Care Package (HCP) funding. You’ll thrive in this role if you’re a confident and self-motivated nurse who enjoys working autonomously, values teamwork, and is passionate about empowering clients to live safely and independently in their own homes.

What you will be responsible for

As a Community Registered Nurse – Care Manager, you will play a critical role in: 

  • Completing initial assessments to identify and addressing client needs based on funding and support requirements. 
  • Developing complex care plans and coordinating service delivery with internal and external stakeholders. 
  • Providing nursing and complex care services, including onsite assessment and support for high-care clients. 
  • Offering clinical advice to staff and clients, supporting safe and quality care. 
  • Monitoring client notes, incident reports and service trends to ensure high standards of care. 
  • Responding to client concerns, incidents or feedback with professionalism and compassion. 
  • Providing coaching and training to support workers and checking competency levels as they deliver care. 
  • Collaborating with families, support workers and allied health professionals to support ongoing client wellbeing. 

 

Skills & Qualifications

To succeed in this role, you will bring: 

  • A tertiary qualification in Nursing (must be AHPRA registered) 
  • A strong understanding of the NDIS Practice Standards, person-centred planning, and risk management. 
  • A valid Australian Driver Licence is essential, as the role involves community visits and supporting clients in their homes.
  • Exceptional interpersonal and communication skills, with a focus on collaboration and respect. 
  • Strong organisational and decision-making abilities, especially in fast-paced environments. 
  • A genuine commitment to improving the lives of people living with disabilities.
