About the Role

What you will be responsible for

The Care Coordinator is pivotal to ensuring the successful delivery of services to our clients. Reporting directly to a Team Leader, your core responsibility will be the organisation, scheduling, and management of support workers to maintain consistent and high-quality support. You will contribute to the team’s success by: 

  • Coordinating and actioning client service requests and bookings in line with Leora’s policies and guidelines 
  • Managing feedback, complaints, and incidents from clients and support workers 
  • Monitoring client funding and renewal processes 
  • Allocating, coordinating, and deploying rosters in alignment with the SCHADS Award and client budgets 
  • Building strong relationships through regular communication with clients, support worker, families, and other stakeholders 
  • Handling calls and emails effectively and in a timely manner 

Skills & Qualifications

To excel in this role, you will bring: 

  • Previous experience in a related role, preferably within the disability or aged care sectors 
  • Qualifications in Health, Disability, Aged, or Community Care 
  • Exceptional communication and relationship-building skills 
  • A strong ability to problem-solve and manage multiple competing priorities in a fast-paced environment 
  • Knowledge of the National Disability Insurance Scheme (NDIS) is highly desirable
