Self-managed aged care gives you more say over how your Support at Home services are arranged.
You may want to choose your own support workers, cleaners, allied health providers or other approved services, while still having a registered aged care provider help with care planning, compliance, claims, payments and ongoing oversight.

With Leora, self-management is not about doing everything alone. It is about having more choice and control, with the right support behind you.

Even with self-managed care, your approved care provider is legally responsible for holding your aged care funds and ensuring that your care and spending comply with government guidelines and standards. However, you will pay lower fees compared to a fully managed Support at Home budget.

Important things to know before you self-manage your aged care supports at home with Leora

Who will I speak to at Leora?

You will have a Care Partners your main point of contact. They can help you understand your care plan, provider arrangements, budget updates and any changes that come up along the way.

We charge up to 10% overhead on each third-party provider invoice. This covers things like provider onboarding, compliance oversight, claiming and payment processing.

Separately, under the Support at Home program, 10% of your quarterly budget is set aside for care management if you receive ongoing services. This is a program requirement and is allocated to your provider’s care management account.

We will explain any fees before services begin, so you know what will be charged to your budget.

If you choose to receive support from Leora Support Workers, you will be charged our regular Support Worker rates.

There is no additional third-party overhead because the support is delivered directly by our team.

This may include cleaning, gardening, nursing, allied health, physiotherapy, occupational therapy and other approved Support at Home services.

Some services may be delivered directly by us, while others may be arranged through partner or subcontracted providers, depending on your approved supports and what you need.

How much does self-managed aged care cost?

We charge a flat rate for self-managed care. Your services and the level of support you may choose to receive from Leora will be tailored to your individual needs. Contact our team so we can discuss your needs and provide a personalised quote.

The new Support at Home program, beginning in November 2025, is the next step in improving in-home care. It replaces the Home Care Packages (HCP) and Short-Term Restorative Care (STRC) programmes, making access to care simpler, faster and more flexible. 

What’s new: 

  • Funding is managed through quarterly budgets rather than yearly packages. 
  • A defined service list shows exactly what’s included. 
  • Care management funding is set at 10% of each quarterly budget. 
  • A new Assistive Technology and Home Modifications (AT-HM) Scheme supports equipment and home adjustments. 

If your needs change, you can be re-assessed and moved to a higher classification under Support at Home.
