What is the Assistive Technology and Home Modifications (AT-HM) Scheme?

The Assistive Technology and Home Modifications (AT-HM) Scheme will operate as one of the short-term care pathways within the Support at Home program. It has its own funding tiers, rules and supports to make access to essential equipment and home changes easier for you.
How are Assistive Technology and Home Modifications (AT-HM) changing under the Support at Home program?
The Australian Government’s new Support at Home program began on 1 November 2025, bringing one of the biggest updates to aged care in years.
A major part of this change is the AT-HM Scheme, designed to make it quicker, simpler and more affordable for you to access the equipment and home modifications you need to stay safe and independent at home.
Under the old system, many older Australians had to use their care budget or save over time for essential items. With the new scheme now in place, you receive dedicated funding for assistive technology and home modifications based on your assessed needs, not on your budget or savings.
How the AT-HM funding will work
The government has introduced a three-tier funding structure for the AT-HM Scheme:
- Low cost (up to $500) – for simple, lower-cost supports
- Medium cost (up to $2,000) – for more tailored equipment or modifications
- High cost (up to $15,000) – for larger or more complex modifications or assistive technology
Important
- High-tier funding is not capped at $15,000.
- If your equipment costs more, you can access additional funding by providing supporting evidence such as a prescription.
- Complex home modifications can receive up to 24 months of funding, initially 12 months with an optional extension if supporting evidence is provided.
- Funding is time limited.
- You must spend the funds within 12 months of signing your service agreement. Funds that remain after that period cannot be used.
An aged care assessor will recommend the appropriate funding tier using the Integrated Assessment Tool (IAT), based on your functional ability, goals and home environment.
If approved, the funding tier and type of support will be listed in your Notice of Decision and support plan. Funding for assistive technology and home modifications is separate from the budget for other Support at Home services.
If you are moving from a Home Care Package (HCP)
You will automatically have access to the AT-HM Scheme.
You can also use any Commonwealth unspent funds from your HCP to buy assistive technology or home modifications. This applies as you move into the new Support at Home system.
