Leora Healthcare’s hospital to home assistance ensures you have the support you need in place to get settled at home after surgery or a lengthy hospital stay.
Are you being discharged from the hospital and need support?
Coming home after a hospital stay can feel a little overwhelming, especially if you don’t have family or friends living nearby, or anyone available to lend a hand.
Leora can guide you home. We can coordinate your care to ensure a smooth, stress-free transition, providing you support from the moment you’re discharged through to helping you temporarily at home as you recover.
If you require more permanent or ongoing support, we can also help you find disability accommodation or assist you with specific NDIS, aged care or attendant care services.

Supporting Your Smooth Transition
We’ve been providing care to the community since 2008, working with clinicians and allied health providers to ensure patients are supported from their hospital bed to their home.
We’re happy to host a complimentary discovery session where we can meet with you individually, or with your wider healthcare team, to find out how we can best support you.

With decades of experience, we know the power of collaboration. We’ll work seamlessly with your healthcare team—including occupational therapists, speech pathologists and behaviour support practitioners—to create a care plan that works for you.
We understand the complexities of higher-level care needs due to a disability such as an acquired brain injury, spinal cord injury, cerebral palsy, epilepsy, or a complex mental health condition. Our support workers receive this specialist training from registered nurses.
Your safety and well-being come first. Beyond meeting the best practices set by the NDIS and Aged Care Commissions, Leora is proudly ISO 9001 certified. This means we meet a globally benchmarked quality standard to ensure you receive the safest, most reliable care every day.
No call centres, just real people who know your name. Enjoy a dedicated Care Coordinator who is your go-to person at Leora Healthcare. We call when we say we will, and we answer your questions without the jargon.
