About the role
Are you the kind of professional who thrives where others hesitate? Do you have the grit, skill, and empathy to work with people living with complex behaviours, mental health challenges, and psychosocial disabilities? If so, we want you on our team.
At Leora Healthcare, we don’t just tick boxes, we change lives. Our Care Partners are at the frontline of person-centred support, designing and delivering tailored care plans that actually work. We’re looking for someone who’s more than experienced – you’re confident, resilient, and relentless about making sure clients get the best shot at independence, dignity, and choice.
What You’ll Do
- Oversee the operations of multiple SIL and SDA sites while also being the go-to advocate for clients and families guiding them through NDIS pathways, housing options, and funding applications.
- Develop, implement and regularly review individualised care plans of clients across disability, aged care or injury-recovery needs
- Coordinate service requests with Care Coordinators to ensure timely, appropriate responses.
- Ensure effective implementation of Behaviour Support Plans (BSPs) by training and coaching staff to support people with complex needs, while reviewing and providing feedback to specialists.
- Navigate the tricky world of mental health and disability with compassion, creativity, and confidence.
- Train and coach support workers so every shift is safe, consistent, and person centred.
- Monitor risks and maintain a safe environment by ensuring compliance with policies, procedures, and standards.
What We're Looking For
- Background in mental health, disability, behaviour support and overseeing SIL/SDA homes – you’ve done the hard yards and you’re not afraid of complexity.
- Strong grasp of NDIS frameworks and compliance requirements.
- A calm but assertive presence when working with clients who may be distressed, aggressive, or at risk.
- A natural problem-solver who can think on your feet and adapt fast.
- Allied health, aged care, or disability qualifications (bonus points if you’ve worked across multiple funding streams).
