From 1 November 2025, Support at Home will replace the Home Care Package program. Whether you’re an existing Home Care Package recipient or new to Support at Home, we’re here to guide you!
What is Support at Home?
Support at Home gives older Australians easier access to services that support their day-to-day living, health and social connection. This includes services such as nursing care, personal care, meal preparation, transport, help around the home and access to mobility aids or equipment. A Care Partner from Leora Healthcare can work closely with you to design a care plan tailored especially for your needs.

The new Support at Home program, beginning in November 2025, is the next step in improving in-home care. It replaces the Home Care Packages (HCP) and Short-Term Restorative Care (STRC) programmes, making access to care simpler, faster and more flexible.
What’s new:
- Funding is managed through quarterly budgets rather than yearly packages.
- A defined service list shows exactly what’s included.
- Care management funding is set at 10% of each quarterly budget.
- A new Assistive Technology and Home Modifications (AT-HM) Scheme supports equipment and home adjustments.
If your needs change, you can be re-assessed and moved to a higher classification under Support at Home.
The Support at Home program officially begins on 1 November 2025. If you currently receive Home Care or Short-Term Restorative Care, your services will transition smoothly with no gap in support.
If you currently receive services under the Commonwealth Home Support Programme (CHSP), you’ll remain in that programme for now. CHSP services will move to Support at Home no earlier than July 2027.
If you’re assessed for more comprehensive care before then, you may start receiving services under Support at Home sooner.
Yes, absolutely. You can pause your Support at Home services for reasons such as a hospital stay, family visit or holiday. Your quarterly budget will continue, although carry-over limits apply. If you don’t use your funding for four consecutive quarters (around a year), your unspent funds may be reallocated, so it’s best to let your Care Partner know if you’ll be away for a while.
Yes! Leora supports self-management for participants who want more control over their care. You can choose your own workers, manage your schedule and decide how your budget is spent with guidance from your Care Partner whenever you need it.
Your costs depend on your funding level and income. The government contributes a subsidy based on your assessed needs, and you may be asked to make a small contribution if you’re able. At Leora, we’ll always provide a clear, upfront breakdown before you begin services. No hidden fees, ever.

An aged care assessment helps determine what support you’re eligible for under the Support at Home program. An assessor from My Aged Care will meet with you (in person or by phone) to discuss your health, home life and goals.
This will help them understand what kind of help will make everyday life easier for you. You can also have a registered supporter, family member or friend present during your assessment.
The Support at Home Priority System ensures that people with the greatest needs receive care first.
It looks at factors such as your health, living situation and level of risk, not just how long you’ve been on the waiting list.
This makes the process fairer, transparent and focused on urgency.
Wellness and reablement are the key principles of the Support at Home program. They emphasise maintaining or regaining independence through goal-focused, short-term support.
Care providers work alongside participants to build strength, confidence and everyday capability, supporting people to stay active and connected.

If you receive support under the Disability Support for Older Australians (DSOA) program, you can continue receiving disability supports even if you move into other programs such as aged care. These supports can work alongside aged care services to ensure you continue receiving the right level of care as your needs change.
If you’re an NDIS participant and you joined the scheme before turning 65, you can continue receiving your NDIS supports for as long as you remain eligible.
Once you turn 65, you can choose to:
- Stay with the NDIS and keep receiving your existing disability supports, or
- Transition to aged care programs, such as Support at Home, if that better meets your needs
